I'm not sure that a Self Service Report is the correct choice for this kind of report. I'm not aware of a way to collapse multiple levels of a Part BOM into one a flat list of parts using only Self Service Reporting.
It is possible to create a copy of the existing BOM Quantity Roll-up report, and then change the Stylesheet to edit how the report is displayed to make it fit your use case.
