Note that version 14+ utilizes .NET core instead of .NET framework.  There is a pretty good chance that you have implemented some custom methods that are not compatible with .NET core.  I would add one more bullet point to update those methods.  We moved from 11sp9 to 15 and that caused a fair amount of pain -- even as a subscriber utilizing the upgrade team.
