I haven´t seen your exact described behavior, but similar refresh issue. You seem to have give an explanation already.
You have changed workflow and Lifecycle. But have you also updated the transition handler Method?
The Express ECO is one of the most complex features in Innovator. It gets very unstable and buggy as soon as you change it like the way you would customize other ItemTypes.

Add the RefreshImpactGridForceServerCall method to the form you have setup for Express ECO as a Form Event as seen below:
Cool that you were able to find this one! I completely have forgotten how many elements were required for the Impact Matrix.
Once you mastered the Impact Matrix, every other task appears easy. And you have better knowledge then 2/3 of the professional consulters out there. :-)
