I haven´t seen your exact described behavior, but similar refresh issue. You seem to have give an explanation already.
You have changed workflow and Lifecycle. But have you also updated the transition handler Method?
The Express ECO is one of the most complex features in Innovator. It gets very unstable and buggy as soon as you change it like the way you would customize other ItemTypes.
Thank you for the reply, I resolved the issue by adding the refresh method to my new form. The one in question is the RefreshImpactGridForceServerCall
