I want to create a form similar to the Part promote. On click of the promote, how the form is showing the available life cycle state ? How this is achieved.

The promotion dialog is not stored as a Form inside of Aras Innovator. It's instead an HTML file that you can find in the code tree under \Innovator\Client\scripts\promoteDialog.html.
You can use this HTML file as an example to build a custom dialog yourself. You can then refer to the last section of this blog post for sample code on how to open this custom HTML page within a dialog.
