Attaching same file to Affected item and DCO on release

trying to attach same file on affected document and dco on release of the dco. getting below error   File "file name" is selected more than once: from "Express DCO name" by "Express DCO File(related_id)", from "Document Name" by "Document File(related_id)". Please contact to admin to resolve conflict. How to ressolve this conflict.
